Accident: Pacific Blue B738 at Brisbane on Oct 14th 2011, stairs toppled by engine exhaust of B744

By Simon Hradecky, created Thursday, Oct 20th 2011 13:34Z, last updated Thursday, Oct 20th 2011 13:34Z

A Pacific Blue Boeing 737-800 on behalf of Virgin Australia, registration VH-VUM performing flight DJ-4197 from Brisbane,QL (Australia) to Denpasar Bali (Indonesia), was preparing for departure from the international terminal at Brisbane around 10:00L (00:00Z) with the first officer conducting his walk around to inspect the aircraft and walking over the mobile aluminium stairs providing access to the Boeing 737.

A Qantas Boeing 747-400, registration VH-OEH performing flight QF-8 from Brisbane,QL to Sydney,NS (Australia), was taxiing for departure at that time, when the engine exhaust of the Jumbo blew the mobile stairs attached to the 737 over sending the first officer down onto the tarmac, the first officer received a broken leg and a broken arm.

The first officer was taken to a local hospital.

Brisbane Airport said it was the first occurrence of that kind that they are aware of. The Airport is working with both operators to determine how that accident happened.

Virgin Australia reported an aluminium staircase was blown over by the exhaust of a taxiing Qantas Boeing 747-400 causing the first officer standing on the stairs to fall onto the tarmac. The first officer received a broken arm and a broken leg. "Our plane was in the right place at the right time" the airline continued stating the aircraft was parked normally in its usual parking bay.

Qantas said, their crew was not using excessive thrust but applying normal standard operating procedures following instructions by Air Traffic Control, when they passed the Virgin Boeing 737 on their way to departure.

The Australian Transportation Safety Board said, they are not investigating the occurrence because they expected little gain from an investigation.

The Virgin Boeing 737 reached Denpasar Bali with a delay of 90 minutes as result of the occurrence.

L'ATSB ha rilasciato nel proprio bollettino le risultanze dell'indagine su questo evento. AvHerald riporta:

"On Mar 14th 2012 the ATSB summarized the event in their bulletin stating that VH-VUM was parked at Bay 76B in direct line with holding point C9. The first officer had been tasked to compute the fuel required for the flight to Denpasar and went to communicate that figure with the refueller exiting the aircraft via the left rear door onto aluminium push stairs, the investigation was not able to determine whether the locking pads enabling the stair to sustain winds of up to 50 knots had been applied correctly.

At that time the VH-OEH, lightly loaded with a takeoff weight of 234 tons (max takeoff weight 412 tons), had been holding at point C9 and was cleared to turn left onto taxiway B. Data off the flight data recorder showed the peak power used to initiate taxi of VH-OEH to holding point C9 as well as from holding point C9 was significantly higher than on previous flights as was the peak taxi speed.

The ATSB reported that Virgin Australia took an immediate safety action disallowing the use of aluminium stairs at bay 76B. Brisbane airport took an immediate safety action removing intermediate holding points C9 and C10 from use and requiring all aircraft to exit the international apron without stop and with minimum power until on the taxiway."
